Accident Summary:
A light fire team departed the 176th AHC to escort a CH-54, which was scheduled for three (3) lifts between Tam Ky and Tien Phouc. During the final escort mission, the aircraft commander of aircraft 64-14165 noticed a slight yaw to the right and simultaneously heard a "popping" noise from the area of the engine compartment. Suspecting a compressor stall, the aircraft commander took the controls and lowered the collective to reduce power. The engine continued to run rough and the torque guage indicated zero (0) pounds. Suspecting a low side governor position and attempted to increase power (throttle and collective) manually. The N1 rose from 65% to approximately 80% but began to decrease rapidly. The N2 rpm started to dissipate but the torque remained at zero (0) throughout the procedure. The aircraft commander selected a clearing secured by a few APC's for the autorotative touchdown. During the progress of the autorotation, the UH-1C was slowed to avoid the CH-54 making an approach to the same LZ. Execution of the flare at 300 feet AGL coupled with the high gross weight caused the UH-1C to develop a high vertical rate of descent. The aircraft impacted in a level attitude atop a small hill in a slightly sloped cultivated field. Upon impact the aircraft bounced and rocked forward causing the retreating blade to strike the ground. The blade strike caused the aircraft to yaw 90 degrees to the left, roll on its right side, and come to rest that position.

Accident Summary:
Burning aircraft sighted from air after impact. Investigation revealed non-standard bolts and nuts for installation of aircraft universal stores rack. Loss of top bolt caused other points to fail. XM-16 subsystem pivoted down and rearward striking tailboom which came off in flight.
