Accident Summary:
Pilot in command was taking off from a 3 foot hover wiht a fully loaded UH-1M aircraft, with 1450 pounds of fuel. While entering translational lift, the aircraft was caught in the turbulence from the aircraft in front of it. The left front skid caught in the PSP creating damage to both skids and crosstubes. The aircraft was flown back to Chu Lai and provisions were made to land the aircraft safely.

Accident Summary:
On 14 August 1971 aircraft, UH-1H, 723 departed Chu Lai at about 08:30 hours on a resupply mission. The aircraft had flown for about seven hours and had refueled four times with no mishaps. At about 14:00 hours the aircraft was refueled for the fourth time at Tien Phuoc. Shortly after 1400 hours the aircraft was on a normal approach to LZ West. At about 20 knots, 50 to 100 feet and 30 pounds of torque the aircraft began to bleed RPM. In an effort to gain RPM the aircraft commander lowered the pitch and tried to land short of the pad. At this time the aircraft no longer responded to pedal control. On touchdown the left skid of the aircraft caught on the elevated PSP pad and began to rock backward and to the right. The aircraft commander then pulled the remaining power in an attempt to pick up on the pad. The anti-torque control was completely negative and the aircraft spun off the pad. The main rotor struck the ground and the aircraft continued backward uncontrolled. The aircraft came to a stop about 40 meters from the pad facing 180 degrees to the approach path.
